    Our goal is to empower individuals and families so that they can make the right decisions concerning issues effecting their life.  We work to support you and do not expect any payment for our services.

    How we can help you?

    At your request we can:

    • Meet/speak with any providers on your behalf with your consent only

    • Work to connect you with service providers to better support you and your family

    • Work for your family's well being by accompanying you to service providers, including school meetings, courts, lawyers, department of social services, etc.

    • Offer 1 on 1 mutual support

    • Benefits advice

    • Wellness Recovery Action Planning

    • Educate you to make informed decisions on your own.

    • Connect you to your passions within our community.

    • Search for information such as"State Education Laws, mental health laws, and school district policies to be sure that families are being treated in accordance with the law and/or policy.

    Who is eligible for our services?

    Any past/present recipient of mental health services or individuals dealing with:

    • anxiety

    • depression

    • hopelessness

    • isolation

    • abuse

    • poverty

    • homelessness

    • problems pursuing dreams/goals

    • loss of liberty through institutionalization

    •  major life disruption

    • trauma

    How to schedule an appointment:

    You must call ahead to schedule appointments, we are unable to take walk in appointments as we have other individuals who request our services.  Phone calls will be returned within 24 hours. CALL 518-521-3507

    Here at Community Connections we are a safe space and environment for Lesbian, Gay, Bisexual,  Transgender, Gender Queer/Non Conforming, Intersex community members, family members and allies 18+ years of age to meet and enjoying being themselves.

    Based Waiver Services

    HCBS peer supported services are peer-delivered services with a rehabilitation and recovery focus. They are designed to promote skills for coping with and managing behavioral health symptoms while facilitating the utilization of natural resources and the enhancement of recovery-oriented principles (e.g. hope and self efficacy, and community living skills.) Peer support uses trauma-informed, non clinical assistance to achieve long term recovery from a behavioral health disorder. These services are non clinical, trauma informed, and community based focusing on the 8 dimensions of health such as;

    • Emotional

    • Environmental

    • Financial

    • Intellectual

    • Occupational

    • Physical

    • Social

    • Spiritual

    Parents Supporting

    Parents 

    Parents Supporting Parents is a peer driven support group to address the challenges and feelings families face while working with;

    • Schools

    • Department of Social Services

    • Probation

    • Behavior Health

    • Community Support

     

     

    We offer hope through shared experiences, resources and education to empower parents to address those feelings. Please join this group to share and support other parents with  similar situations, concerns, and feelings in a safe, non-judgmental environment.

    Have you or anyone you know needed someone to talk to? Someone to actively listen without interruption or judgement? Would you like someone to call you and ask how your day went? Our WARM LINE provides you with someone who has lived experience and understands the difficulties that are faced daily.
