Health Home Care Management
Our care management program assists adult individuals with identifying and accessing services and/or supports that will assist them in addressing problems they are experiencing or that will help them to achieve a higher quality life or level of functioning. The role of the Care Manager includes, but not limited to, assisting an individual in developing or practicing skills (or confidence) that will help them to be able to use the resources and services available to them as independently as possible. The services and/or supports that are offered through this program are assisting clients with multiple significant disorders (physical, mental health, and/or substance abuse, as well as troubles within their social determinants of health) that interfere with their current level of functioning through comprehensive care management, care coordination & health promotion, comprehensive transitional care, individual & family support, and connection to community & social support services.
